For Becton Dickinson’s PROVE program, a “Big Picture” was developed that visualizes the path to change management for patient pathway adaptation. It illustrates how analysis, evaluation of baseline data, patient pathway development, training, and implementation work together to adapt clinical processes in a transparent manner and integrate monitoring solutions.

Initial Situation & Challenge: Making Complex Patient Pathways Easier to Understand

The global healthcare system faces significant challenges in various areas. These include, among other things, waiting lists for elective surgeries, capacity bottlenecks in intensive care units, postoperative complications, longer hospital stays, and rising costs for inpatient care. The PROVE program addresses these interrelated issues. The goal is to tailor individual treatment pathways and help hospitals better organize their treatment processes. The challenge was to present this process in a clear and understandable way: from the initial analysis and mapping of existing patient pathways to the implementation of tailored processes in everyday clinical practice. The “Big Picture” was intended to illustrate how many stakeholders, data sources, process steps, and clinical areas are interconnected—and why a structured view of the patient pathway is important in this context.

The Solution: A Big Picture Approach to Customizing the Patient Journey

The Big Picture translates the PROVE program’s workflow into a visual structure. It shows how the analysis of existing data can lead to a concrete project: starting with an evaluation of baseline data, followed by joint coordination, the development of a customized patient pathway, training of the project team, and subsequent implementation. This makes it clear that it is not just about individual measures. What is crucial is the connection between clinical data, existing processes, BD Advanced Patient Monitoring products, training, and practical implementation in routine hospital care. The diagram provides guidance for the groups involved and shows how the operating room, intensive care unit, general ward, and hospital administration are integrated into the overall process.
Individual scenes
Based on clinical data and existing patient pathways, a treatment pathway is developed in collaboration with the hospital team. In this process, protocols and influencing factors are coordinated so that subsequent changes can be evaluated in a measurable way.
The adapted patient pathway is being implemented in everyday clinical practice. As part of this process, new monitoring procedures are being supported, potential implementation gaps are being identified, and relevant data is being collected to evaluate the adaptation based on defined influencing factors.
After surgery, key data on the patients' condition is collected to enable retrospective analysis and to make it possible to measure the effects along the patient pathway.

This is how the big picture came about

Working with the central point of contact, we gathered the relevant content, organized it, and developed an initial structure. Building on this foundation, we gradually created a big-picture view that clearly visualizes the technical relationships and illustrates the added value for the customer.
1. Content & Structure
To begin with, the key project phases, terms, and departments involved were compiled. This resulted in an initial content structure that makes the process easy to follow.
2. Development & iteration
Based on this, the content was broken down into individual scenes. Care was taken to ensure that technical concepts were presented in an understandable way and that the individual steps within the overall process remained clear.
3. Feedback & Finalization
In the final step, the content, terminology, and scenes were reviewed and refined collaboratively. The concluding feedback from management helped refine the project to its final version.

Effect & Results

The Big Picture helps make the PROVE program easy to understand and connects the various project phases. It illustrates how clinical data, patient pathways, training, and implementation work together, and which contacts and teams within the hospital are involved. This makes it possible to measure the added value of BD Advancen Patient Monitoring and its products more quickly, and ensures that this value is sustained for all stakeholders along the patient pathway.
